Shirley P. Middleton, M. D. P. C is a solo practice providing comprehensive eye care including treatment of eye diseases and injuries, cataract, glaucoma and laser surgery, diabetic eye care, Low Vision Aids, etc.
This practice is affiliated with New View Laser Eye Dr. Jacqueline D. Griffith for laser refractive surgery.

Insurance Plans:
We accept most major medical health insurance plans including but not limited to Medicare, DC Medicaid, Blue Cross/Blue Shield, Care First, MDIPA, NCPPO, Aetna, Aetna US Healthcare, Avesis, Advantica, Cigna, Davis Vision, Health Right, Quality Plan, United HealthCare, Mamsi, Bravo Health, Core Source, Mailhandlers, GEHA, Optimum Choice, Unicare, Vision Benefit of America, Fep Blue, and more.
Call the office and/or your insurance plan to determine our participation.
(Some medical insurances DO NOT Cover routine eye examinations and refractions)
Many insurances require referrals from your primary care physician.

Monday thru Friday: 7:00 to 4:00
Patient Hours: By appointment.
Monday and Thursday: By appointment. Please call office.
Tuesday, Wednesday and Friday: 7:30 to 1:00; hours by appointment. Please call office.

Patient age: 9 years old and up
Payment: Cash. Check, Charge (American Express, Visa, Discovery, Master Card)

Co-pay & Deductibles: Expected at the time of services.
We will gladly submit to your insurance.
Emergency: For a sight-threatening emergency, go directly to an emergency room.
You may contact this office thru the answering service at 866-851-1170.
Prescription refill information: Have the pharmacy contact the office by phone or Fax for authorization for medication refills. You may also call the office and provide the name of the medication, the medication number and the pharmacy phone and or fax number and we will call or FAX the refill to the pharmacy.
